Electronic Visa Application Form
The Department of State’s electronic visa form site allows you to complete your visa application form online in English or in French. Form DS-156 is for all applicants.
All your information is entered by you on-line and at your convenience. When complete, you will print your application form, which will include a special 3rd page with an electronic barcode. At the time of your visa interview, bring with you both the printed form and the barcode page. Consular Section personnel will then scan in your information using the barcode. This time-saving method is convenient for applicants and more accurate. This service is offered in English and several other languages. The site can be reached at http://evisaforms.state.gov
Men between the ages of 16 and 45 as well as persons born in or bearing national passports of Iran, North Korea, Cuba, Sudan, Syria and Libya must ALSO fill out the supplementary form DS-157.

(You must have Adobe Acrobat Reader version 5.0 or higher installed on your PC in order to download and print the completed application form. If you do not have Adobe Acrobat Reader version 5.0 or higher, you may download a copy from Adobe's web site.)
As of November 1, 2006, all applicants must complete the DS-156 on-line using the site http://evisaforms.state.gov and bring the 3 completed pages with them to the interview. Application forms DS-156 completed by hand or typed will no longer be accepted.
Men between the ages of 16 and 45 as well as persons born in or bearing passports of Iran, North Korea, Cuba, Sudan, Syria and Libya must, in addition to form DS-156, fill out the supplementary form DS-157: DS-157-English DS-157Eng.pdf. This form will be accepted when completed by hand or using this PDF form.
Photograph
All Non-Immigrant Visa applications (DS-156) must have a photograph attached. This photograph must measure 50 millimeters X 50 millimeters (a square). The photograph must be taken in front of a white or off-white background. Photographs will be digitally scanned so there are no exceptions to the size or background color requirements.
